I am aware that I will have to pay monthly, the amount of R $ 25.00 (Twenty-Five Reais) as an Associative Contribution with maturity every 10th (Dec) of each month.
I am aware that this monthly contribution amount includes a Personal Accident Insurance Policy contracted by AMACRE from Porto Seguro Cia de Seguros Gerais, whose General Conditions are available on the website www.portoseguro.com.br
As an associate, I will lose the Insurance benefit, with the non-payment of 01 (One) contribution and the other benefits earned by AMACRE, with 03 (Three) consecutive contributions.
- I declare for all legal purposes that I know the AMACRE Constitution Statute and its objectives, I undertake to send copies of RG, CPF, CNH and Proof of Address, which will be part of this register.
